Expert consensus on irrigation and intracanal medication in root canal therapy.

化学的洗浄と消毒は、根管治療における感染除去のための重要なステップである。しかし、潅注剤の選択や潅注の手順は明確とは言い難い。根尖部におけるベーパーロック効果は未だ解決されておらず、灌流の有効性を阻害し、残留感染や治療成績の低下を招いている。さらに、曖昧な根管内投薬の臨床的適応と標準化されていないドレッシング・プロトコルを明確にする必要がある。不適切な根管内投薬は副作用を引き起こし、治療結果を危険にさらす可能性がある。実際、臨床家は長年にわたってこのような懸念を認識してきた。本稿では、現在の研究エビデンスに基づき、さまざまな潅注剤と管腔内薬剤の特性をレビューし、それらの有効性と相互作用を明らかにする。また、さまざまな運動学的潅注法の進化、その効果、限界、パラダイムシフト、現在の適応症、および管腔内投薬に関する効果的な操作手順についても論じている。本専門家コンセンサスは、根管灌流の臨床操作ガイドラインおよび根管内投薬に関するポジション・ステートメントを確立することで、感染制御に対する理解を深め、臨床実践を標準化し、最終的に歯内療法の成功を向上させることを目的としている。

Chemical cleaning and disinfection are crucial steps for eliminating infection in root canal treatment. However, irrigant selection or irrigation procedures are far from clear. The vapor lock effect in the apical region has yet to be solved, impeding irrigation efficacy and resulting in residual infections and compromised treatment outcomes. Additionally, ambiguous clinical indications for root canal medication and non-standardized dressing protocols must be clarified. Inappropriate intracanal medication may present side effects and jeopardize the therapeutic outcomes. Indeed, clinicians have been aware of these concerns for years. Based on the current evidence of studies, this article reviews the properties of various irrigants and intracanal medicaments and elucidates their effectiveness and interactions. The evolution of different kinetic irrigation methods, their effects, limitations, the paradigm shift, current indications, and effective operational procedures regarding intracanal medication are also discussed. This expert consensus aims to establish the clinical operation guidelines for root canal irrigation and a position statement on intracanal medication, thus facilitating a better understanding of infection control, standardizing clinical practice, and ultimately improving the success of endodontic therapy.
