:後天性免疫不全症候群（AIDS）患者におけるサイトメガロウイルス網膜炎（CMVR）に続発する網膜剥離（RD）に対する硝子体手術の臨床的特徴と関連する予後因子を分析する。この研究では、硝子体手術を受けた39眼が対象となりました。最良修正視力（BCVA）、CD4細胞数、網膜癒着率、予後因子を術後1、3、6ヵ月後に評価しました。1ヵ月後の網膜癒着率（87.2％）は、3ヵ月後（82.1％）、6ヵ月後（71.8％）よりも高かった。6ヵ月後の再発RDと有意に関連する因子は、CD4細胞数＜50cells/μL（=.000）と弛緩網膜切開（=.002）であった。6ヵ月後の視力(≤logMAR1.0)の予後と有意に関連する因子は、帯状角膜症( =.015)とRDによる黄斑病変( =.012)であった。CD4細胞数が50cells/μL未満の場合と弛緩網膜切開術は予後の悪化と関連し、帯状角化症とRDによる黄斑病変は視力の低下と関連している。

: To analyze the clinical characteristics and relevant prognostic factors of vitrectomy for retinal detachment (RD) secondary to cytomegalovirus retinitis (CMVR) in patients with acquired immune deficiency syndrome (AIDS).: This study involved 39 eyes that underwent vitrectomy. Best-corrected visual acuity (BCVA), CD4 cell counts, retinal attachment rate, and prognostic factors were assessed 1, 3, and 6 months postoperatively.: The rate of retinal attachment at 1 month (87.2%) was higher than at 3 (82.1%) and 6 (71.8%) months. Factors significantly related with recurrent RD at 6 months were CD4 cell count <50 cell/µL ( = .000) and relaxed retinotomy ( = .002). Factors significantly related with the visual (≤logMAR1.0) prognosis at 6 months include band keratopathy ( = .015) and macular involvement by the RD ( = .012).: CD4 cell counts <50 cell/µL and relaxed retinotomy are associated with worse prognosis, while band keratopathy and macular involvement by the RD are related to poorer vision.
