トロンボ炎症とは、心血管疾患において重要な役割を果たす血液凝固と炎症の複雑な相互作用のことです。第3回マーストリヒト血栓症コンセンサス会議では、基礎、トランスレーショナル、臨床の研究者が集まり、心筋梗塞、脳卒中、末梢動脈疾患などの心血管疾患患者の病因、診断、管理における血栓性炎症の起源と潜在的な影響について議論した。本論文では、以下のトピックに関する専門家の意見やコンセンサス勧告を最先端に反映させたものを紹介する。(1)内皮細胞バリアの課題、(2)血小板、好中球、好中球の細胞外トラップに焦点を当てた循環細胞と血栓性炎症、(3)プロコアグラントのメカニズム、(4)アテローム性動脈硬化症における動脈血管の変化。アテローム性動脈硬化と虚血・再灌流傷害の減衰；(5)動脈血管疾患患者の管理；(6)静脈血栓症の病態と静脈血栓塞栓症の晩期障害。
Thrombo-inflammation describes the complex interplay between blood coagulation and inflammation that plays a critical role in cardiovascular diseases. The third Maastricht Consensus Conference on Thrombosis assembled basic, translational, and clinical scientists to discuss the origin and potential consequences of thrombo-inflammation in the etiology, diagnostics, and management of patients with cardiovascular disease, including myocardial infarction, stroke, and peripheral artery disease. This article presents a state-of-the-art reflection of expert opinions and consensus recommendations regarding the following topics: (1) challenges of the endothelial cell barrier; (2) circulating cells and thrombo-inflammation, focused on platelets, neutrophils, and neutrophil extracellular traps; (3) procoagulant mechanisms; (4) arterial vascular changes in atherogenesis; attenuating atherosclerosis and ischemia/reperfusion injury; (5) management of patients with arterial vascular disease; and (6) pathogenesis of venous thrombosis and late consequences of venous thromboembolism.
