Long-term outcome in patients after treatment for Cushing's disease in childhood.

はじめに:
クッシング病(CD)は高コルチゾール血症の稀な原因であり、診断および治療上の大きな課題となっている。小児期のCD治療後の長期追跡における下垂体機能に関するデータは限られている。
INTRODUCTION: Cushing's disease (CD) is a rare cause of hypercortisolemia presenting a major diagnostic and therapeutic challenge. Data on pituitary function in long-term follow-up after CD treatment in childhood is limited.

小児記念健康研究所(CMHI)の小児期CD治療後の患者さんの長期評価。
AIM: Long-term assessment of patients of the Children's Memorial Health Institute (CMHI) after CD treatment in childhood.
材料と方法:
CD患者29人のレトロスペクティブ解析、診断時の平均年齢13.46歳。長期追跡調査(FU)は以下の方法で行われた。1)患者の問診票からデータを得る(成人患者の75%)、2)問診票に回答しなかった患者と現在のCMHI患者の最終診察時のデータを使用する、という方法で行った。経蝶形骨膜下垂体手術(TSS)による平均長期FUは10.23年であった。
MATERIALS AND METHODS: Retrospective analysis of 29 CD patients, mean age at the time of diagnosis 13.46 yrs. The long-term follow-up (FU) was done by: 1) obtaining the data from a patient's questionnaire (75% of adult patients); 2) using the data from the last clinic visit for patients who did not respond to the questionnaire and for current CMHI patients. The average long-term FU from transsphenoidal pituitary surgery (TSS) was 10.23 yrs.
結果:
最新のFUでは、TSS1後に18例(62%)、TSS2後に2例(6.9%)、TSS後放射線治療(XRT)サイクル後に1例(3.4%)、両側副腎摘出術(BA)後に3例(10.3%)の長期寛解が認められた。1例(3.4%)は術後合併症によりTSS2後に死亡、1例(3.4%)は最新のFU時に病変が持続し、1例(3.4%)は長期FUができなかった。CD再発は28例中4例(14%)に発生し、最終治療から平均3.6年であった。BA後の1例(3.4%)はNelson症候群のために手術を受けた。2例(6.9%)は最新の評価時に再発が疑われた。最終評価時には、確定治療後レボチロキシン治療17例(63%)、ヒドロコルチゾン治療16例(59%)、性ホルモン補充剤10例(37%)、抗利尿ホルモン4例(15%)が投与されていた。
RESULTS: At the latest FU: 18 patients (62%) had long-term disease remission after TSS1, 2 patients (6.9%) after TSS2, 1 patient (3.4%) after the post-TSS radiotherapy (XRT) cycle and 3 patients (10.3%) after bilateral adrenalectomy (BA). One patient (3.4%) died after TSS2 due to postoperative complications, 1 patient (3.4%) had persistent disease at latest FU, in 1 patient (3.4%) the long-term FU was not possible to perform. CD recurrence occurred in 4 out of 28 patients (14%) at an average time 3.6 yrs. from definitive treatment. One patient (3.4%) after BA was operated because of Nelson's syndrome. Two patients (6.9%) were suspected of relapse at latest assessment. At the time of the last evaluation, 17 patients (63%) were on levothyroxine therapy since definitive treatment, 16 patients (59%) were on hydrocortisone treatment, 10 patients (37%) were taking sex hormones replacement, 4 patients (15%)-antidiuretic hormone.
結論:
小児期にCD治療を受けた患者の中には、ホルモン性下垂体障害に加えて気分障害や認知障害を有する患者が比較的多い。効果的な治療後、長期間経過しても再発することがあります。
CONCLUSIONS: Relatively large number of patients after CD treatment in childhood have hormonal pituitary deficits as well as mood and cognitive disorders. CD recurrence can occur even after a long time post effective treatment.
