背景:
アルジェリアでは、Rickettsia conorii conoriiによって引き起こされ、Rhipicephalus sanguineusによって感染する地中海斑点熱(MSF)が流行している。しかし、実験施設が不足しているため、症例の記録は稀である。我々の目的は、発熱、発疹、接種の可能性のあるエッシャーを持つ患者を対象に、リケッチア症のスクリーニングを行うことであった。
BACKGROUND: In Algeria, Mediterranean spotted fever (MSF), caused by Rickettsia conorii conorii and transmitted by Rhipicephalus sanguineus, is endemic. However, the documentation of cases is rare due to a lack of laboratory facilities. Our aim was to screen for rickettsioses in patients with fever, rash and a possible inoculation eschar.
材料と方法:
2013年から2015年の間に、Tizi-Ouzou地域の病院で発熱と発疹を呈した患者をプロスペクティブに本研究の対象とした。間接免疫蛍光アッセイ(IFA)を用いて血清をスクリーニングし、エスカルスのスワブサンプルにqPCRを実施した。
MATERIALS AND METHODS: Between 2013 and 2015, patients with a fever and a rash presenting at hospitals in the Tizi-Ouzou region were prospectively included in our study. Sera were screened using Indirect Immunofluorescence Assay (IFA) and qPCR was performed on swab samples from eschars.
結果:
1606人の患者が含まれていた。57 例の患者について、R. conorii conorii に起因する MSF は、スワブエッシャーサンプルの血清学と qPCR により診断された。3名の患者はノミ媒介性リケッチ症の一つであるマウスチフスと診断された.また,2名の患者ではR. felisのIgMが陽性であった.9人の患者では、リケッチ症の血清検査が陽性であったが、特定のリケッチ症を特定することはできなかった。9人の患者は神経学的病変または多臓器不全を伴う重症であった。
RESULTS: One hundred and sixty-six patients were included. For 57 patients, MSF due to R. conorii conorii was diagnosed by serology and qPCR on a swab eschar sample. Three patients were diagnosed with murine typhus, a flea borne rickettsiosis. In addition, two patients had a positive serology in IgM for R. felis. For nine patients, serology for rickettsiosis was positive, but the specific rickettsia involved could not be determined. Nine patients had a severe disease with neurological involvement or multi-organ failure.
結論:
アルジェリアからの帰国時に発疹を呈した患者や旅行者に対しては、臨床医は日常的にリケッチ症のスクリーニングを行うべきである。予後が重篤になる可能性があるため、ドキシサイクリンを速やかに投与する必要がある。
CONCLUSION: Clinicians should routinely screen for rickettsioses in patients and travellers presenting with a rash upon return from Algeria. Doxycycline treatment must be given promptly because the prognosis can be severe.
