Cost-effectiveness of orthodontics: a systematic review.

背景:

医療経済評価は、健康上の利益を最大化し、機会費用を最小化するための方法論であり、医療における資源配分の決定に情報を提供する上で重要性が増している。

BACKGROUND: Health economic evaluation is a methodology to maximize health benefits and minimize opportunity costs and is of increasing importance in informing resource allocation decisions in healthcare.

目的:

この系統的レビューの目的は、あらゆる歯列矯正介入に関する経済評価の可用性と質を評価し、これらの研究の結論を要約することである。

OBJECTIVE: This systematic review aims to assess the availability and quality of economic evaluations of any orthodontic interventions and summarize the conclusions of these studies.

検索方法:

2000 年 1 月から 2022 年 2 月までの間に、歯列矯正治療と経済評価に関連する用語を含む文献を 徹底的に検索した。MEDLINE、EMBASE、SCOPUS、Web of Science、NHS Economic Evaluation Database、Cochrane を検索した。灰色文献を検索し、関連するシステマティックレビューの参考文献リストをさらに手作業で検索した。

SEARCH METHODS: A thorough search of the literature was carried out including terms related to orthodontic interventions and economic evaluation between January 2000 and February 2022. MEDLINE, EMBASE, SCOPUS, Web of Science, NHS Economic Evaluation Database, and Cochrane were searched. Grey literature was searched and further hand-searching was performed on the reference lists of relevant systematic reviews.

選択基準:

口唇口蓋裂手術と睡眠時無呼吸症候群に関する研究は、これらの疾患が多分野にまたがるため、歯列矯正の介入への結果の適用性が変わる可能性があるため除外した。2人の独立したレビュアーが、包含する研究を選択した。

SELECTION CRITERIA: Studies on cleft lip and palate surgery and sleep apnoea were excluded due to the multi-disciplinary nature of these conditions which might alter the applicability of the result to orthodontic interventions. Two independent reviewers selected studies for inclusion.

データ収集と分析:

データ抽出は、2人の著者による対象研究の全文分析によって行われた。各研究の質は、ドラモンド10点チェックリストとNational Institute for Health and Care Excellence（NICE）の経済評価のための品質評価チェックリストに従って評価した。対象論文の特徴および結論は、記述的にまとめた。

DATA COLLECTION AND ANALYSIS: Data extraction was carried out through full text analysis of included studies by two authors. The quality of each study was assessed according to the Drummond 10-point Checklist and the National Institute for Health and Care Excellence (NICE) Quality Appraisal Checklist for Economic Evaluations. Characteristics and conclusions of included articles were descriptively summarized.

結果:

16編の論文が組み入れ基準を満たした。ほとんどの論文が臨床試験ベースの研究であり、モデルベースの研究は 1 件のみであった。評価された歯列矯正介入は、反対咬合矯正、機能的装置治療、顎骨整形手術であった。ほとんどの研究はヨーロッパ、特にスウェーデンで実施された。研究の質は一般的に低く、NICEチェックリストによると69％の研究が重大な制限を有すると報告されている。

RESULTS: Sixteen articles met inclusion criteria. Most of the included articles were trial-based studies, with only one model-based study. Orthodontic interventions assessed included crossbite correction, functional appliance treatment and orthognathic surgery. Most studies were carried out in Europe and specifically in Sweden. The quality of included studies was generally low with 69% of studies reported as having serious limitations according to the NICE Checklist.

結論:

このレビューでは、歯列矯正の介入に関する経済評価の欠如と、既存の経済評価の限界を強調した。今後の研究についての推奨を行う。

CONCLUSION: This review highlighted a lack of economic evaluations for orthodontic interventions, and limitations of existing economic evaluations. Recommendations on future research are provided.
