目的:

3つの異なる2段式口蓋裂修復法による歯列弓関係の結果を比較すること。

OBJECTIVE: To compare dental arch relationship outcomes following 3 different 2-stage palatal repair protocols.

デザイン:

レトロスペクティブ、クロスセクショナル

DESIGN: Retrospective, cross sectional.

セット:

日本の3つの口蓋裂センター（A，B，C）。

SETTING: Three cleft palate centers (A, B, C) in Japan.

対象者:

日本人の完全一側性口唇口蓋裂患者90名（A：39名、B：26名、C：25名）が連続して治療を受けた。

PATIENTS: Ninety (A: 39, B: 26, C: 25) consecutively treated Japanese patients with complete unilateral cleft lip and palate.

介入方法:

Aでは、平均1歳7ヵ月で軟口蓋と硬口蓋の後半分を修復した。Bでは、軟口蓋と硬口蓋は、それぞれ平均1歳6ヶ月と5歳8ヶ月で別々に閉鎖された。Cでは、軟口蓋と硬口蓋は、それぞれ平均1歳と1歳5か月で閉じられた。

INTERVENTIONS: In A, the soft palate and the posterior half of the hard palate were repaired at a mean age of 1 year 7 months. In B, the soft palate and hard palate were closed separately at a mean age of 1 year 6 months and 5 years 8 months, respectively. In C, the soft palate and hard palate were closed at a mean age of 1 year and 1 year 5 months, respectively.

主要評価項目:

5人の評価者による5歳児（5-Y）指数と2人の評価者によるHuddart/Bodenham（HB）指数を用いて、歯列弓の関係を評価した。

MAIN OUTCOME MEASURES: Dental arch relationships were assessed using the 5-Year-Olds' (5-Y) index by 5 raters and the Huddart/Bodenham (HB) index by 2 raters.

結果:

評価者間信頼性は、5-YとHBの評価において、ほぼ完全に一致した。5-Yスコアの平均値および分布は、3施設間で有意な差は認められなかった。マイナーセグメントの臼歯のHBインデックススコアの平均値は、CではAおよびBよりも有意に小さかった（< 0.05）。

RESULTS: Intra- and inter-rater reliabilities showed substantial or almost perfect agreement for the 5-Y and HB ratings. No significant differences in mean values and distributions of 5-Y scores were found among the 3 centers. The mean HB index scores of molars on the minor segment were significantly smaller in C than those in A and B ( < .05).

結論:

硬口蓋閉鎖術の時期と術式によって、5年後の歯列関係に有意な差はなかった。しかし、共変量としての乳児の裂け目の大きさの影響の可能性については、より大きなサンプルサイズでのさらなる分析が必要である。

CONCLUSIONS: There were no significant differences in dental arch relationships at 5 years among the times and techniques of hard palate closure. However, further analysis of the possible influence of infant cleft size as a covariable on a larger sample size is needed.
