全身麻酔下で治療を受けた小児のPreformed Zirconia Crownsの生存率に影響を与える要因について

Factors Influencing the Survival of Preformed Zirconia Crowns in Children Treated under General Anesthesia.

目的:

本研究は、小児の前歯に装着されたジルコニアクラウンの成功率をレトロスペクティブに評価し、歯の歯髄治療が失敗率に与える影響を評価することを目的とした。

Aim: This study aimed to retrospectively evaluate the success of zirconia crowns placed in the anterior teeth of children and evaluate the impact of pulp therapy of the tooth on the rate of failure.

材料と方法:

全身麻酔下でジルコニアクラウンを装着した3歳から5歳までの小児20名の前歯70本を24ヵ月間追跡調査した。2年間の生存期間を推定するために，Kaplan-Meier生存曲線を描いた．歯髄治療を受けた歯と受けていない歯の結果を比較した。

Materials and Methods: A total of 70 anterior teeth of 20 children aged between 3 and 5 years who had undergone the placement of zirconia crowns under general anesthesia were followed up for 24 months. Kaplan-Meier Survival curves were plotted for the estimation of two-year survival time. The outcomes for teeth that had received pulp therapy were compared to those that had not received pulp therapy.

結果:

70個の歯冠を2年間観察したKaplan-Meier生存率分析の結果、歯冠の平均生存期間は38.7ヵ月で、信頼区間は38.1ヵ月から39.3ヵ月であった。症状の有無に基づいてクラウンの生存期間を観察したところ，2年後の時点で症状（クラウンロスの有無）があったのは70個のクラウンのうち4個のみで，成功率は94.3％であった。また、平均生存期間は39.5ヵ月（信頼区間39.15〜39.98ヵ月）に延長されました。

Results: Kaplan-Meier survival analysis of 70 crowns observed over a two-year period showed that mean survival time for the crowns was 38.7 months with a confidence interval ranging from 38.1 months to 39.3 months. When the survival of the crowns was observed based on the presence or absence of symptoms, it was observed that only 4 out of the 70 crowns were symptomatic (with or without crown loss) at the end of two years, giving a success rate of 94.3%. The mean survival time was also increased to 39.5 months (confidence interval 39.15-39.98 months).

結論:

ジルコニアクラウンは、許容できるレベルの成功率と生存期間を提供する。歯髄治療後の歯に装着されたクラウンは、歯髄治療を行っていない歯に装着されたクラウンよりも失敗する可能性が高い。

Conclusion: Zirconia crowns provide an acceptable level of success and longevity. Crowns placed on teeth after pulp therapy are more likely to fail than those placed on teeth without pulp therapy.
