歯科用インプラント埋入における静的サージカルガイドと比較したダイナミックナビゲーションの正確性

Accuracy of dynamic navigation compared to static surgical guide for dental implant placement.

目的:

歯科インプラント埋入におけるダイナミックナビゲーション（DN）とスタティックサージカルガイド（SSG）の精度を比較し、術者の経験やインプラント部位などの影響因子について検討する。

OBJECTIVES: To compare the accuracy of dynamic navigation (DN) with a static surgical guide (SSG) for dental implant placement and the influence factors such as the experience of the surgeon and the implant sites.

方法と材料:

ダイナミックナビゲーションを実施した38本のインプラントと、スタティックサージカルガイドを実施した57本のインプラントが、レトロスペクティブスタディに登録された。DN群とSSG群では、冠状動脈偏位、骨端部偏位、角度偏位が比較され、DN群では術者の経験値やインプラントの部位が異なることが示された。

METHODS AND MATERIALS: A total of 38 implants, which underwent the dynamic navigation, and 57 implants which underwent a static surgical guide were enrolled in the retrospective study. Coronal deviation, apical deviation, and angular deviation were compared between the DN and SSG groups, along with the different experience level of surgeons and implant sites in the DN group.

結果:

DN群とSSG群、DN群における術者の経験値やインプラント部位に統計的な有意差はなかった。しかし、前歯部ではDNのapical deviationがSSG群より若干高く（P = 0.028）、臼歯部ではDNのangular deviationはSSG群より小さかった。

RESULTS: There were no statistically significant differences between the DN and SSG groups, and the experience level of the surgeons and implant sites in the DN group. However, the apical deviation of the DN was slightly higher than the SSG group in the anterior teeth (P = 0.028), and the angular deviation of DN was smaller than the SSG group in the molar.

結論:

ダイナミックナビゲーションは、スタティックサージカルガイドと同様に正確なインプラント埋入を実現できる。また、術者の経験値やインプラント部位はダイナミックナビゲーションの精度に影響しないが、大臼歯ではDNの精度がSSGより高いようである。

CONCLUSION: Dynamic navigation can achieve accurate implant placement as well as the static surgical guide. Additionally, the experience level of the surgeon and implant site do not influence the accuracy of dynamic navigation, while the accuracy of DN seems higher than the SSG in molar.
