台湾の多発性硬化症患者におけるフィンゴリモドの安全性と有効性に関する実際のエビデンス
Real-world evidence on the safety and effectiveness of fingolimod in patients with multiple sclerosis from Taiwan.

背景・目的:
多発性硬化症は台湾では希少疾患に分類されている。本研究では、台湾の日常診療における再発寛解性多発性硬化症(RRMS)患者を対象に、フィンゴリモドの安全性と有効性を評価した。
BACKGROUND/PURPOSE: Multiple sclerosis is classified as a rare disease in Taiwan. This study evaluated the safety and effectiveness of fingolimod in patients with relapsing-remitting multiple sclerosis (RRMS) from routine clinical practice in Taiwan.
方法:
このレトロスペクティブ多施設観察研究では、2012年9月から2015年12月までの間に日常臨床でフィンゴリモッド0.5mg/日投与を受けた患者の臨床データを収集した。一次アウトカムはフィンゴリモドの全体的な安全性、二次アウトカムは年率換算再発率(ARR)とした。
METHODS: In this retrospective, multicentre, observational study, we collected clinical data of patients treated with fingolimod 0.5 mg/day in routine clinical practice between September 2012 and December 2015. Primary outcome was the overall safety of fingolimod; secondary outcome was the annualized relapse rate (ARR).
結果:
全体では、データ収集期間終了時までに62/69人(86.1%)の患者がフィンゴリモドを服用していた。平均年齢(±標準偏差[SD])は37.7±10.10歳、MSの平均罹患期間は5.4±4.52年、フィゴリモドの平均服用期間は135.8年であった。主な有害事象は、徐脈(21.7%、初回投与時関連)、上気道感染症、めまい、麻酔低下(しびれ)(各11.6%)、次いで尿路感染症、腰痛(各7.2%)であった。肝酵素関連のAEは7例であった。8人の患者で試験期間中の絶対リンパ球数が0.2×10/uL未満であった。1名の患者が初回投与後に第2度AVブロックを発症しました。重篤なAEは11例(15.9%;軽度から中等度)に認められました。新たに発症した黄斑浮腫は認められなかった。ARRはフィンゴリモド投与群で0.3±0.74であり,66.7%の患者が無再発であった。拡張障害状態尺度スコアのベースラインからの平均変化率(SD)は-0.30±1.353であった。
RESULTS: Overall, 62/69 (86.1%) patients were on fingolimod by the end of data collection period. Mean age (±standard deviation [SD]) at inclusion was 37.7 ± 10.10 years; mean duration of MS was 5.4 ± 4.52 years and mean duration of fingolimod exposure was 135.8 patient-years. The most common adverse events (AEs) were bradycardia (21.7%; first-dose related), upper respiratory tract infection, dizziness, and hypoaesthesia (numbness) (11.6% each), followed by urinary tract infection and back pain (7.2% each). Seven patients had liver enzyme-related AEs. Eight patients had absolute lymphocyte counts <0.2 × 10/uL over the study period. One patient developed second degree AV block after first-dosing. Serious AEs were observed in 11 patients (15.9%; mild-to-moderate). No newly developed macular oedema was detected. The ARR was 0.3 ± 0.74 in fingolimod-treated patients and 66.7% of patients were relapse-free. The mean (SD) change from baseline in expanded disability status scale score was -0.30 ± 1.353.
結論:
台湾のRRMS患者を対象に、フィンゴリモド0.5mg/日を平均2年間投与することで、管理可能な安全性プロファイルが得られ、有効性が維持・改善された。
CONCLUSION: Fingolimod 0.5 mg/day treatment with an average of 2 years of exposure was associated with a manageable safety profile, and maintained/improved effectiveness in RRMS patients from Taiwan.
