抄録

背景:

固定薬疹は、表皮内CD8 T細胞介在性薬物反応の特徴的な形態であり、違反薬物の投与を受けた後、同じ部位に単発性または多発性の皮膚病変が繰り返し出現する。非ステロイド性抗炎症薬（NSAID）が最も一般的な原因である。誘導性シクロオキシゲナーゼ2（COX-2）の選択的阻害剤は、従来のNSAIDよりも少量のアレルギー性または特異的な副作用を誘発するが、可変重症度の皮膚反応を引き起こす可能性がある。

BACKGROUND: Fixed drug eruption is a characteristic form of intraepidermal CD8 T cell-mediated drug reaction, with repeated appearance of isolated or multiple skin lesions in the same location after receiving the offending drug. Nonsteroidal anti-inflammatory drugs (NSAID) are the most common cause. Selective inhibitors of inducible cyclooxygenase 2 (COX-2) provoke a smaller amount of allergic or idiosyncratic adverse reactions than conventional NSAID, but can cause skin reactions of variable severity.

OBJECTIVE: Etoricoxib has been related to a variety of unusual skin reactions, including several reports on fixed drug eruption.

方法:

結果etoricoxib誘発性固定性発疹の新規症例7例を紹介する．残存病変では2例でcelecoxib（ペットでは10％）によるパッチテストが陰性であり，1例では経口耐性が確認された。

METHODS: Results: We present seven new cases of etoricoxib-induced fixed eruption, with a diagnosis based on clinical presentation, and confirmed by an etoricoxib-positive lesional patch test in six of them; and by a positive low-dose oral challenge in the other one. Two patients showed negative patch tests with celecoxib (10% in pet) on the residual lesions, and oral tolerance was confirmed in one.
