Predicting Depression Symptoms in Families at Risk for Depression: Interrelations of Posterior EEG Alpha and Religion/Spirituality.

背景:
大うつ病(MDD)の臨床転帰のバイオマーカーとして、脳波後αが同定されている。また、宗教やスピリチュアリティ(R/S)の個人的な重要性は、MDDの家族性リスクが高い人に保護効果をもたらすことが示されている。本研究では、うつ病の臨床的な健康状態の転帰を予測する上で、ポストアルファーとR/Sの共同の価値を直接検討した。
BACKGROUND: Posterior EEG alpha has been identified as a putative biomarker of clinical outcomes in major depression (MDD). Separately, personal importance of religion and spirituality (R/S) has been shown to provide protective benefits for individuals at high familial risk for MDD. This study directly explored the joint value of posterior alpha and R/S on predicting clinical health outcomes of depression.
方法:
混合効果モデルアプローチを用いて、25年にわたる5つのタイムポイントで収集された縦断的データを用いて、21歳時のR/Sの仮想的な推定値を得た。72チャンネルの安静時脳波(目を開けたまま/閉じたまま)の事後αを定量化するために、電流源密度と周波数主成分分析を用いた。うつ病の重症度は、脳波収集後5年から10年の間にPHQ-9とIDAS-GDスケールを用いて測定した。
METHODS: Using a mixed-effects model approach, we obtained virtual estimates of R/S at age 21 using longitudinal data collected at 5 timepoints spanning 25 years. Current source density and frequency principal component analysis was used to quantify posterior alpha in 72-channel resting EEG (eyes open/closed). Depression severity was measured between 5 and 10 years after EEG collection using PHQ-9 and IDAS-GD scales.
結果:
R/Sの増加(p=.008、η=0.076)とαの増加(p=.02、η=0.056)は、尺度間で症状の減少と別々に関連していた。しかし、αとR/Sの間の交互作用(p=.02、η=0.062)が観察され、R/Sが高いと低αでは症状の減少が予測されたが、αが高いと低R/Sでは症状の減少が予測された。
RESULTS: Greater R/S (p = .008, η = 0.076) and higher alpha (p = .02, η = 0.056) were separately associated with fewer symptoms across scales. However, an interaction between alpha and R/S (p = .02, η = 0.062) was observed, where greater R/S predicted fewer symptoms with low alpha but high alpha predicted fewer symptoms with lower R/S.

小から中程度の効果サイズとサンプルの人口統計学の同質性は、全体的な解釈と一般化可能性に注意を促した。
LIMITATIONS: Small-to-medium effect sizes and homogeneity of sample demographics caution overall interpretation and generalizability.
結論:
その結果、R/Sとαの相補的な役割が明らかになり、どちらか一方の変数が低レベルの場合にのみ保護効果を発揮することが明らかになった。これらの知見は、うつ病症状の重症度の予測因子としてのR/S重要度とα振動の関連性を確認した。MDDにおけるR/S重要性の保護効果の基礎となる神経生物学的メカニズムについては、さらなる研究が必要である。
CONCLUSIONS: Findings revealed a complementary role of R/S and alpha in that either variable exerted protective effects only if the other was present at low levels. These findings confirm the relevance of R/S importance and alpha oscillations as predictors of depression symptom severity. More research is needed on the neurobiological mechanism underlying the protective effects of R/S importance for MDD.
