思春期前の女児における膣出血の症例報告と文献レビュー

抄録

思春期前の女の子の膣出血は、複雑な病因を持つ珍しい疾患である。排便後に膣出血を認めて病院に搬送された6歳女児の症例を報告する。生殖器検査や補助検査で様々な共通原因が除外された。膣鏡検査の結果、非エストロゲン化した子宮頸部と膣壁内に点状出血が増加していた。これらの点状疱疹の病因は、腹圧の上昇による小血管または粘膜下毛細血管の破裂によるものと考えられた。結論。この症例報告および文献のレビューは、思春期前出血の病因に新たな光を当て、関連する疾患の評価および適切な治療レジメンの策定において婦人科医が使用するための困難な臨床的特徴を詳述している。

Vaginal bleeding in the prepubertal girl is an uncommon condition with complex etiological factors. We report a case of a 6-year-old girl brought to hospital when vaginal bleeding was noticed after defecation. Various common causes were excluded by genital and auxiliary examinations. Serial vaginoscopy revealed increasing petechiae within the non-estrogenized cervix and vaginal wall. The etiology of these petechiae was considered to result from ruptured small blood vessels or submucosal capillaries due to an increase in abdominal pressure. Conclusion: This case report and review of literature has shed new light on the etiology of prepubertal bleeding, which details challenging clinical characteristics of use to gynecologists in the evaluation of related conditions and formulation of appropriate treatment regimens.
