目的:
スポーツ関連脳震盪(SRC)後に良性発作性発作性体位性眩暈(BPPV)と診断された小児患者の臨床所見と転帰について報告する。
OBJECTIVE: Report the clinical findings and outcomes among pediatric patients diagnosed with benign paroxysmal positional vertigo (BPPV) after sports-related concussion (SRC).

回顧的症例シリーズ。
DESIGN: Retrospective case series.
設定:
集学的小児脳震盪プログラム。
SETTING: Multidisciplinary pediatric concussion program.
患者:
19歳未満でスポーツやレクリエーション活動に関連した脳震盪を起こした患者は、包括的な前庭理学療法評価のために紹介される。
PATIENTS: Patients younger than 19 years with a sport or recreation activity-related concussion referred for comprehensive vestibular physiotherapy assessment.
主要評価項目:
ターゲット粒子リポジショニング(PR)後の症状の消失。
MAIN OUTCOME MEASURE: Symptom resolution after targeted particle repositioning (PR).
結果:
研究期間中に115名の小児SRC患者が前庭理学療法を受け、そのうち12名(10.4%)がBPPVと診断された。片側性後半円管(SCC)BPPVは8/12人(75%)に、片側性前半円管(SCC)BPPVは4/12人(25%)に診断された。良性発作性発作性体位性眩暈は全例で平均1.58回のPRマニューバー(範囲1~4)で治療に成功した。
RESULTS: During the study period, 115 pediatric SRC patients underwent vestibular physiotherapy assessment including 12 (10.4%) who were diagnosed with BPPV. Unilateral posterior semicircular canal (SCC) BPPV was diagnosed in 8/12 (75%) patients, and unilateral anterior SCC BPPV diagnosed in 4/12 (25%) patients. Benign paroxysmal positional vertigo was successfully treated in all patients with a mean of 1.58 targeted PR maneuvers (range = 1-4).
結論:
小児SRCの包括的な管理には、持続的な打撲後症状の異種病態生理に対処するための集学的アプローチが必要である。BPPVを併発している小児SRC患者は、ターゲットとなるPRを考慮すべきである。
CONCLUSIONS: Comprehensive management of pediatric SRC requires a multidisciplinary approach to address the heterogeneous pathophysiology of persistent postconcussion symptoms. Pediatric SRC patients with coexisting BPPV should be considered for targeted PR.
