:縫合不要のバイオプロテーゼは現在、特に高齢者や大動脈狭窄症のある患者の大動脈弁置換術に使用されており、虚血時間を短縮できるという利点がある。我々は、大動脈の逆流と上行大動脈の拡張を有する患者で、弁を温存する手技が術中に失敗した後、Perceval無縫合バイオプロテーゼが植え込まれた症例を報告する。この症例は、3年後の追跡調査で安定した大動脈不全の症例において、無縫合プロテーゼの適応外使用が有効であることを示している。

: Sutureless bioprostheses are currently employed for aortic valve replacement especially in elderly or compromised patients with aortic stenosis having the advantage of reducing the ischemic time. We report a patient with aortic regurgitation and dilatation of the ascending aorta in whom a Perceval sutureless bioprosthesis was implanted after intraoperative failure of a valve-sparing procedure. This case demonstrates an effective off-label use of a sutureless prosthesis in case of aortic insufficiency with stability at 3-year follow-up.
