抄録

背景:

ヘリコバクター・ピロリは、胃や十二指腸に生息する緩徐増殖性の微好気性グラム陰性菌です。また、多くの胃-十二指腸疾患と関連しています。

BACKGROUND: Helicobacter pylori is a slow-growing micro-aerophilic gram-negative organism found in stomach and duodenum. It is also associated with a number of stomach-duodenal diseases.

材料と方法:

ヘリコバクター・ピロリ感染症の検出には、侵襲的な方法と非侵襲的な方法があります。本研究では、13ヶ月間に生検や迅速ウレアーゼ検査、培養検査で感染が確認された101名の患者を対象とした。

MATERIAL AND METHODS: There are invasive and non-invasive methods for detecting Helicobacter pylori infections. In a 13- months period, 101 patients with clinical signs of infection confirmed by biopsy and Rapid Urease test as well as culture were included to this study.

結果:

呼気ウレアーゼ検査、抗ヘリコバクター・ピロリ菌血清IgGおよびIgA、糞便ヘリコバクター・ピロリ菌Agと金標準法との間には有意な相関が認められた（P=0.001）。

RESULTS: There were significant correlations between breath urease test, anti-Helicobacter pylori serum IgG and IgA as well as fecal Helicobacter pylori Ag with gold standard method,(P=0.001) Moreover, there was significant correlation between monocytosis (P= 0.05) and gold standard method.

結論:

ヘリコバクター・ピロリ感染症患者を対象とした研究から、ヘリコバクター感染症の診断には、抗ヘリコバクター・ピロリ菌血清IgMの評価よりも非侵襲的な診断法が有用であり、また、血中単球数の増加が検出の確実な確認となる可能性があります。さらに、感染率の高い地域では、最初の段階で単球症をヘリコバクター・ピロリ感染症として考慮する必要があることが示された。

CONCLUSION: Based on studies conducted on patients with Helicobacter pylori infection, noninvasive diagnostics methods can be useful in the diagnosis of Helicobacter infections rather than evaluating anti-Helicobacter pylori serum IgM and also increased blood monocytes could be a reliable confirmation for detection. Furthermore, showed monocytosis must be considered as a Helicobacter pylori infection at first step in area with high infected percentage.
