下顎インプラント支持固定人工歯に対向する総義歯下の上顎の変化

Maxillary changes under complete dentures opposing mandibular implant-supported fixed prostheses.

目的:

本研究の目的は、下顎インプラント支持固定式人工歯に上顎総義歯を対合させてリハビリを行った患者において、コンビネーション症候群に類似した病態が生じるか否かを明らかにすることである。

PURPOSE: The aim of this study was to determine whether a condition similar to Combination Syndrome occurs in patients rehabilitated with a maxillary complete denture opposing a mandibular implant-supported fixed prosthesis.

材料と方法:

シドニーにあるUnited Dental Hospitalのインプラントセンターの患者プールから、これらの条件を満たした11名の無歯顎患者を対象に、標準化された臨床処置により適合性、咬合整合性、上顎前歯部の骨欠損を測定した。

MATERIALS AND METHODS: Standardized clinical procedures measured fit, occlusal integrity, and bone loss in the anterior maxilla in 11 edentulous subjects meeting these requirements, from the patient pool of the Implant Centre, United Dental Hospital, Sydney.

結果:

上顎前歯の隆起高さの年間平均損失は0.17mmであり、統計学的に有意ではなかった（P > 0.05）。しかし、咬合時の上顎前歯部隆起の圧力の増加、および後方咬合位での咬合接触の喪失は、すべての被験者で片側または両側に認められた。

RESULTS: A mean annual loss of 0.17 mm in the anterior maxillary ridge height was not statistically significant (P > 0.05). However, increased pressure in the anterior maxillary ridge during occlusion and loss of posterior occlusal contacts in retruded position were noted on one or both sides in all subjects.

結論:

後方咬合の喪失は、上顎前歯部の骨量減少とは無関係である。しかし、補綴物およびその支持構造の完全性を維持するためには、定期的なリコール予約を取り、咬合を確認することが重要である。

CONCLUSION: Loss of posterior occlusion could not be related to anterior maxillary bone loss. However, to maintain the integrity of the prostheses and their supporting structures, it is important to schedule periodic recall appointments for review of the occlusion.
