抄録

人工膝蓋大腿関節置換術は、ドイツではまだほとんど行われていない。しかし、厳密な適応と徹底した術前準備があれば、人工膝関節全置換術（TKA）の代替となりうる。保存的治療法が無効であることが判明した場合、単独人工関節置換術の利点が人工関節全置換術の利点を上回るのかどうかという問題だけでなく、膝蓋大腿関節置換術に関しても、インレー人工関節とオンレー人工関節のどちらを使用すべきかという問題が生じる。

Patellofemoral prosthesis procedures are still rarely performed in Germany. However, with strict indications and thorough preoperative preparation, it can be a viable alternative to total knee arthroplasty (TKA). If conservative treatment methods prove ineffective, the question arises not only as to whether the advantages of an isolated joint replacement outweigh those of a total prosthesis, but also regarding patellofemoral joint replacement: should an inlay or an onlay prosthesis be used?

膝蓋大腿関節の人工関節置換術は、ドイツでは以前より行われている。この人工関節は、優れた適応症と術式により、人工膝関節全置換術(KTEP: Kniet Total Endoprothese)の優れた選択肢となります。保守的な治療法が頓挫している今、「腫 瘍形成術に比べ、アイソレーショントレーニングの利点は何なのか」という問いもある：オンレイ・プロテーゼかインレイ・プロテーゼか？

Der patellofemorale Gelenkersatz wird in Deutschland nach wie vor selten durchgeführt. Bei korrekter Indikationsstellung und Operationstechnik stellt dieser eine gute Alternative zur Knietotalendoprothese (KTEP) dar. Zeigen sich konservative Therapieansätze frustran, stellt sich jedoch nicht nur die Frage, ob die Vorteile eines isolierten Gelenkersatzes gegenüber denen einer Vollprothese überwiegen, sondern auch: Onlay- oder Inlay-Prothese?
