喫煙者における実験的インプラント周囲粘膜炎において、トリクロサン含有フッ化物配合歯磨剤をステントに適用した場合の臨床パラメータおよび骨炎症メディエーターに及ぼす影響

Triclosan-containing fluoride toothpaste on clinical parameters and osteo-inflammatory mediators when applied in a stent during experimental peri-implant mucositis in smokers.

目的:

喫煙者を対象とした実験的インプラント周囲粘膜炎において、トリクロサン含有フッ化物配合歯磨剤をステント内に塗布した際の臨床パラメータおよびインプラント周囲液中の骨-免疫炎症メディエーターに及ぼす影響を明らかにすること。

OBJECTIVES: To determine the effect of triclosan-containing fluoride toothpaste on the clinical parameters and the osteo-immunoinflammatory mediators in the peri-implant fluid when applied in a stent during experimental peri-implant mucositis in smokers.

材料と方法:

この二重盲検無作為化クロスオーバー試験には、インプラント歯冠を有する喫煙者26名が登録された。機械的歯ブラシを行わない2回の3週間（洗浄期間：30日）の間、患者は1日3回、トリクロサン/フッ化物配合歯磨剤（n：13）またはフッ化物配合歯磨剤（n：13）に無作為に割り付けられた。ベースライン、3、7、14、21日目に臨床検査および免疫酵素学的検査を行った。

MATERIALS AND METHODS: Twenty-six smokers with an implant-supported crown were enrolled in this double-blind, randomized, crossover study. During the two 3-week periods without mechanical toothbrushing (washout period: 30 days), patients were randomly assigned to triclosan/fluoride (n:13) or fluoride toothpaste (n:13), three times/day. Clinical and immunoenzymatic assays were performed at baseline, 3, 7, 14 and 21 days.

結果:

両群とも試験期間を通じてPlaque Indexの上昇を認めたが（p=0.001）、21日目には群間差は認められなかった（p>0.05）。IFN-γ、IL10、IL-1β、IL8、IL-17、IL-6、TNF-α、MMP-2、MMP-9、TGF-β、OC、OPN、ICTP、OPG、RANKLについては、群内差も群間差も認められなかった（p>0.05）。しかし、RANKL/OPG比は、機械的歯ブラシを行わない期間が終了した21日目に、トリクロサン/フッ化物処理部位と比較してフッ化物歯磨剤処理部位で有意に高かった（p=0.041）。

RESULTS: Both groups showed increase in the Plaque Index throughout the study (p = 0.001), without inter-group differences at 21 days (p > 0.05). No intra- or inter-group differences were observed for IFN-γ, IL10, IL-1β, IL8, IL-17, IL-6, TNF-α, MMP-2, MMP-9, TGF-β, OC, OPN, ICTP, OPG and RANKL (p > 0.05). However, the RANKL/OPG ratio was significantly higher in fluoride toothpaste-treated sites when compared to triclosan/fluoride-treated sites at the end of period without mechanical toothbrushing, on the 21st day (p = 0.041).

結論:

トリクロサン含有歯磨剤は、喫煙者における実験的インプラント周囲粘膜炎において、骨免疫炎症メディエーターを良好に調節し、RANKL/OPG比を低下させた。

CONCLUSION: Triclosan-containing toothpaste favorably modulated osteo-immunoinflammatory mediators during the experimental peri-implant mucositis in smokers, decreasing the ratio of RANKL/OPG.
